Services & Amenities we Offer

Medication Management

 

  • Oversee medication administration in accordance with physician orders 
  • Perform medication reconciliation and review 
  • Monitor for side effects, adverse reactions, and effectiveness 
  • Ensure proper storage, documentation, and compliance with regulations 

On-site nursing services

 

  • Conduct initial and ongoing resident health assessments 
  • Monitor vital signs (blood pressure, pulse, temperature, oxygen levels) 
  • Identify changes in physical, mental, or emotional condition 
  • Communicate findings to physicians, families, and care team 

Three nutritious, well-balanced Meals

 

Meals shall:

  • Meet the basic nutritional needs of residents in accordance with dietary guidelines 
  • Include a balance of: 
    • Protein (meat, poultry, fish, eggs, or alternatives) 
    • Whole grains or starches 
    • Fruits and vegetables 
    • Dairy or calcium-rich options 
  • Be prepared using healthy cooking methods (baked, grilled, steamed vs. fried when possible)

  • Therapeutic or modified diets provided as ordered by a physician (e.g., diabetic, low sodium, pureed)

Assistance with scheduling and coordinating medical appointments

 

  • ssist residents and/or responsible parties with scheduling appointments, including: 
    • Primary care visits 
    • Specialist appointments 
    • Dental, vision, and podiatry services 
    • Therapy services (physical, occupational, speech) 
  • Maintain an organized system (calendar/log) for tracking appointments 

Coordination of Care

  • Communicate with healthcare providers to: 
    • Confirm appointment details 
    • Share necessary medical information (as authorized) 
    • Coordinate referrals and follow-ups 
  • Ensure continuity between provid

transportation to medical appointments

 

  • Assist with arranging transportation to and from appointments, including: 
    • Family or responsible parties 
    • Medical transport services 
    • Facility-approved transportation providers 
  • Ensure residents are prepared for transport (documents, medications, etc.) 

24-hour supervision and support to ensure resident safety and well-being

 

  • Staff are present on-site 24 hours a day, 7 days a week 
  • Awake overnight staff available to monitor residents and respond to needs 
  • Staffing levels are sufficient to meet resident care requirements 

Resident Monitoring & Safety

  • Regular observation of residents to ensure safety and well-being 
  • Monitoring for: 
    • Changes in physical or mental condition 
    • Fall risks or mobility concerns 
    • Behavioral or emotional changes 
  • Implementation of safety checks and rounds, including overnight 

Personalized care plans

 

  • Conduct a comprehensive assessment prior to or upon admission, including: 
    • Medical history and current health status 
    • Cognitive and behavioral status 
    • Functional abilities (ADLs) 
    • Dietary needs and preferences 
    • Social, cultural, and personal preferences 
  • Assessment used to determine level of care and services required 


Housekeeping, laundry, and linen services

 

  • Routine cleaning of resident rooms and common areas, including: 
    • Dusting, vacuuming, and mopping floors 
    • Cleaning and sanitizing bathrooms 
    • Trash removal and disposal 
  • Cleaning schedules maintained daily, weekly, and as needed 
  • Immediate cleaning of spills or hazardous conditions 

 Laundry Services

  • Regular laundering of residents’ personal clothing 
  • Separation of laundry to prevent cross-contamination 
  • Use of appropriate detergents and water temperatures 
  • Prompt return of clean clothing to residents 

Social engagement and recreational activities

 

  • Develop a structured activity program on a weekly or monthly basis 
  • Activities tailored to residents’: 
    • Interests and preferences 
    • Cognitive and physical abilities 
    • Cultural and spiritual backgrounds 
  • Activity calendar posted and accessible to residents and families 

Types of Activities Offered

Activities may include, but are not limited to:

  • Group activities (games, bingo, arts and crafts) 
  • Exercise and movement programs (chair exercises, stretching) 
  • Music and entertainment 
  • Educational programs 

Assistance with communication between residents, families

 

  • Assist residents with making and receiving: 
    • Telephone calls 
    • Video calls (if available) 
    • Written correspondence (letters, emails) 
  • Provide access to communication devices as needed 

Family Notifications

  • Notify family members or responsible parties of: 
    • Significant changes in condition 
    • Medical concerns or emergencies 
    • Hospitalizations or incidents 
  • Communication conducted in accordance with resident consent and privacy regulations

ADL Assistance

 

  • Support provided as needed with: 
  • Bathing, dressing, grooming 
  • Toileting and incontinence care 
  • Mobility and transfers 
  • Eating and hydration 
  • Services delivered in a manner that promotes independence and dignity
